How to fill in the online health form and get the Peruvian health affidavit

Until October 12, 2022 everyone - visitor, resident and Peruvian; adult and child; vaccinated or unvaccinated - who came to Peru by air was required to present a Health Affidavit when entering the country. This changed with the Ministerial Resolution 811-2022-MINSA.

So, since October 12, 2022, the Health Declaration is no longer required to enter Peru! The website to fill in the form has been deactivated as of October 20, 2022.

As Peru is still in a state of (national health) emergency and prevention and control measures to stop the spread of Covid remain in place at least until February 24, 2023, things can be tightened again in no time and the health declaration might be necessary in the future again. So, for now, we leave our guide to fill in the form below.

 

Completing the online form takes less than 10 minutes, is super simple, free of charge and at the end you immediately get the required health affidavit which you can either download or print. The form can be filled in up to 72h before entering Peru (within 72 hours prior to your arrival in Peru). You might be asked to show the health affidavit when checking in to your flight to Peru and/or at the airport in Peru and/or when traveling.

Please only use the official website for obtaining the health affidavit and don’t fall for a number of service providers offering to get the sworn statement for you while charging you anything from US$ 20 up to US$ 60 for something you can easily do yourself in a few minutes for free.

How to complete the online form and get the Peruvian health affidavit

If applicable, before you start switch off your VPN and any translation program that might run on purpose or automatically!!!

Step 1

Visit the official health affidavit website.

There have been reports that the page doesn’t always load correctly in some browsers, an error message pops up and people can't fill in the required fields on the first page. If you are using a VPN, switch it off, reload the page and it should work smoothly. Otherwise, try to use a different browser; we had no problems with Chrome, Firefox and Safari.

Additionally, we get many questions that people can't find nationalities or states or cities in the drop-down lists on the first and second page. Note that the names are always in Spanish (see Step 2)! If you see them translated, switch off any translation program that may run in the background automatically by your browser or intentionally otherwise error messages can pop up or you won't find the correct names.

Step 2

At the bottom of the first page you can choose between English and Spanish, however while the labelling of the spaces you have to fill in changes from Spanish to English and vice versa, the drop-down list under Nationality remains in Spanish; so if you are, for example, a US national you won’t find United States or US on the list, but have to click on EE.UU (Estados Unidos, the Spanish name for the US); or if you are German, click on Alemania, etc. Those from the UK will look in vain for United Kingdom but as well for Reino Unido; select instead Gran Bretaña  (Great Britain) or, if you are from England, you could choose Inglaterra, or, if you are from Scottland, Escocia.

Then choose the document with which you are travelling: DNI, CE or PAS (passport) and enter the number. Complete the first page by adding your birthdate and then click on “Search”.

Step 3

The second page of the Peruvian online form to get the health affidavit is divided into two parts:

  • Part I: Personal Information
  • Part II: Health information

So, first fill in your personal information exactly as in the document you are traveling with, your arrival date and flight number. Under “Country of origin” (the country from which the flight departs to Peru) the names are again in Spanish (see step 2).

Then choose your destination in Peru. You are asked for the department (Peruvian state), the province and the district; so, for example, if you stay in the district of Miraflores in Lima, choose Lima (department), Lima (province) and Miraflores (district) or if you travel to Machu Picchu choose Cusco (department), Urubamba (province), Machu Picchu (district). If you are traveling around, either choose your first destination or the place where you stay the longest.

Now add a contact phone number and a cell phone number. Be aware that fields for the (cell) phone number are Peruvian-style, so, if necessary, you might have to "adjust" your phone number to the Peruvian structure.

  • the country code must have 2 or 3 digits
  • the city / area code must have 2 or 3 digits
  • the phone number must have at least 6 digits, the cell phone number at least 8 digits

Example: let’s assume you have a US cell phone number which usually is structured as follows: +1 (country code) 123 (area code) 1234567 (number). This cell phone number won't be accepted as there are two problems with it: the system doesn’t allow the “+” and the number only has 7 digits. So, just enter in the country code field 01, in the city code field 12 (the first two digits of your area code) and in the number field 31234567 (last digit of your area code and then the 7 digits of your number). Problem solved.

Then enter a valid e-mail address you have access to even during your travel.

The following fields seem to be aimed at Peruvian travellers (and in an earlier version of this form were in a section which only Peruvians had to fill out) and don't really fit international tourists.

You are asked for your "real address", but you can only choose a Peruvian department (state), province and district. So, just as above, where you had to choose your destination in Peru, use the address of your hotel or the address of friends / family you are staying with. If you are traveling around, choose the first hotel you are staying at or the place where you stay the longest.

And the next field seems even stranger; you are asked to enter your "real address after isolation". This just makes little sense for foreign travellers, so just enter the street address of your accommodation.

The next fields aren't better as you should add the persons who are with you at your "real address". We recommend to either leave these fields blank (each person traveling should have his or her own health declaration anyway) or enter the information of whoever is traveling with you. After entering the data of a person, you have to click "Add", otherwise the system won't accept the entry.

Step 4

In Part II Health Information, as of May 2022, you just have to answer three Covid-related questions and then tick the box before the declaration of oath; the geolocation authorization for your cell phone, which never had a legal basis in Peru, is gone.

Click registrar, and you immediately get the Health Affidavit. At the top of the page, you can and, as once you leave this page you can't return, should download ("descargar") and/or print ("imprimir") the form. Sometimes, if you have selected "imprimir" the window that opens remains blank. Just shut the print window with the "x" at the top right, choose the "descargar" button and print from there.

      After clicking on the little calender, click on the triangle next to the year. Then click two times on the arrow pointing to the left. There you can choose the year 1945.

    If I leave blank the data of the person traveling with me because I am traveling ALONE,  do I still  have to click "Add", otherwise the system won't accept the entry or do I not click add??? Very confusing!! Thanks in advance for your help.

      as mentioned above, you can fill in the form up to 72 hours (3 days) before your arrival in Peru. As the form is checked when you check-in to your flight, best fill it out in the time period 72 hours prior to your arrival in Peru and check-in.

    Hello, I submitted/registered the form and once I printed I realized that I didn't change the date of arrival and so it now has the wrong date of my arrival. Am I able to submit again with the correct date?

    I was wondering if we need to put our middle name in the personal information section, and if so where. For example, in one of the "last name" boxes or next to our first name in the "first name" box.

      in Peru we don't have middle names, just one or more first names. So your first name and your middle name belong in the "first name" field. If you only have one surname, write it in the first "last name" field and leave the second "last name" field empty; if you have two, write one in the first and one in the second "last name" field.

      Just make sure that your names are exactly as in your passport.

      no, nothing needed from the Peruvian side.

      You, of course, have to fulfil the requirements of your destination country, which are usually checked by the airlines when leaving Peru.

     Today i completed the Peru Affidavit of Health and Geo location Authorization form. While completing the form, i did not find an option to check the "I accept the geolocation of my declared cell phone" option. And also after the completing the form, i did not receive any email either. Will there be any issue if resubmit the form? Thanks in Advance

      you are right. I just checked it and the "I accept the geolocation of my declared cell phone", which didn't have any legal basis anyway, is gone. Great. Thank you so much for pointing the change out. 

      And you don't get an e-mail after you filled in the form. When you click on "registrar" at the bottom of the page a new window opens. There you see the health affidavit which you must either print or download.

      If you didn't do that, you can fill in the form again.

      you just need the health declaration form for your international flight to enter Peru. 

      So, no, for your national flight from Lima to Cusco you do not need to fill in the form again.

    Correction: Two of my family members are flying from Chicago-Miami-Lima today May 19. They already completed the forms put we get an error message of nginx when we try to print the forms. Please help

      something using the "imprimir" button doesn't work properly. So, do the following: when you completed the form and click on registrar you get the filled in form. On the top you can choose either to print, which sometimes doesn't work (error messages pop up or the page remains blank), or to "descargar" (download). If you click on descargar you can save the form and as well print it. That should work.

    Hi Eva,

    Thank you so much for this site, it is a real breath of fresh air.  I need your advice.  I fly out of Juliaca to Lima and arrive in Lima at 5:20pm.  I fly out of Lima to Panama the next day arriving at 8:11am.  Do I have enough time to obtain an antigen test during this layover for my final destination that requires it?  From reviews I have been reading, it seem near to impossible...  Your advice would be greatly appreciated!  Thank you.

      You sure you only need an antigen test? Then best use Unilabs at the airport. They offer antigen tests and PCR tests at the airport for passengers, which you are, 24/7. You find them in the parking lot. The antigen test is S/ 100, the PCR test S/ 185; results are ready in one hour (antigen) or 12 hours (PCR). The lab asks to please make an appointment here but you can as well just walk in and get the test done.

      Thanks for your response.  

      For my final destination, they require either an antigen or PCR test.  Arrival time is 4:30 the next day which is out side of the PCR 48hr window I would get before boarding the flight from Juliaca the day before...

      What I really need to know is... Will it actually take an hour to receive the result?  Is there a short waiting period?  I leave the next morning at 4:30... So there is little room for error.

      honestly I can't follow anymore. You arrive in Lima from Juliaca at 05.20 pm. So, as the flight takes around 1.5h you leave Juliaca at around 3.45pm. One time you wrote you arrive in Panama the next day at 8.20 am, then arrival time is 4.30, then you leave at 4.30. No matter when you arrive the next day in Panama, from leaving Juliaca to arriving in Panama you are clearly inside the 48h window. Or am I missing something?

      Anyway, having little room for error is never a good thing in Peru, something can always go wrong. So, I don't know how many people will be at the testing center when you arrive. Therefore, best make an appointment under the link I provided above and waiting times will be minimal. And yes, if you take an antigen test you usually have the result in an hour, PCR test results will take up to 12 hours.

    I filled out affidavit of health on 5/16/22.  My flight is May 19th at 5:00pm. The affidavit does not show time of the day I prepared. I do not remember what time I prepared the form.  So I tried to fill out the form again, but it won't let me do it again.  How do you know the affidavit was done within 72 hours?  It only shows date of filing.  Am I missing something?

      no, you don't miss anything, you worry too much.

      If the system allowed you to fill in the form and you filled it in on May 16 (and this date is on your health declaration) arriving in Peru on May 19 (so three days or more or less 72 hours later), you are fine. Nobody, neither when checking in nor when entering Peru will know the exact hour when you filled in the form. So, all good.

    HI. my friend is travelling to Peru on May 20, flight from Florida to Lima to Cusco as the final destination. we cant proceed with the filling up the affidavit. the departure time will be May 20 at 5 pm from Florida USA. what is the best time to file the affidavit? thanks

      the earliest you can fill out the health declaration is 72 hours before your trip to Peru. So, if the flight leaves on May 20 at 05.00 pm exactly now we are 72 hours before. 

      Personally, I would wait until tomorrow or even the day after tomorrow to fill in the form. Then you shouldn't have a problem. Your friend just should make sure to have the health declaration when checking in to the flight.

    Hi,
    Can you advise me again? My travel company documentation says “ complete an affidavit within 72 hours of travel in which you must upload your proof of vaccination “
    well I have completed the affidavit but I could not see anywhere to upload anything. Have I missed something.? I have printed off the affidavit and printed off my vaccination certificates.

      the Peruvian Health declaration website has no option to upload a vaccine certificate. You just need the affidavit either in digital or printed form.

      When you are flying into Peru just after leaving the plane and before immigration there is personnel from the Ministry of Health which checks the Health declaration and the vaccine certificate. That's it.

    Hi there!
    I have two questions: fist I was wondering if Peru recognize the just one Johnson & Johnson vaccine as the the double (2  vaccines) Pfizer vaccinations? I received the only required J&J dose and one Pfizer booster.
    My second question is about how to fill out the cell phone number spaces. I am from the States so for country-code you write 01, then ask for State that here are 3 digit, lastly the rest of the cell # that are 7 numbers, however when I write down the left 7 numbers it comes as incomplete. As the article said to split up the numbers, would t mean to take one State number and place it on te slot for cell number to make it 8, so that the system accepts my entry?       Answer question 1:

      Until the end of March 2022 the Peruvian Covid regulations indicated that you must have “completed your vaccination scheme”, so be fully vaccinated; meaning that those with one shot of the J&J vaccine as well were or, as this never really worked in Peru, should have been considered fully vaccinated.

      This changed at the beginning of April 2022 with an update of the Covid regulations (Supreme Decree 030-2022-PCM) and was confirmed just recently with the new Supreme 041-2022-PCM that is in effect since May 1, 2022. In both regulations the “vaccination scheme” is gone, so there is no “fully vaccinated” anymore. Instead, you now find in the regulations that you must have received your first and second dose of a vaccine against Covid (“primera y segunda dosis de vacunación contra la COVID-19”) regardless which vaccine brand was used (see Supreme Decree 041-2022-PCM, article 4.4).

      So, one shot J&J isn’t what you consider “fully vaccinated”; it’s just one shot. So, having received one shot and a booster is only considered 2 shots.

      Answer question 2:

      Yes, exactly.

      Lets assume your phone is 01 (country code) 123 (city /state code) 1234567 (cell phone number)

      The country code has to have two or three digits.
      The city code as well two or three digits.
      The cell phone number at least eight digits.

      So, you fill in the fields as follows

      Country Code: 01 (US country code, two digits, fine)
      City code: 12 (you need one more digit for the cell phone number so here just the first two digits of your city code)
      Phone number: 31234567 (the first digit is the last digit of your city code, the rest your cell phone number and voila now you have the required eight digits in the cell phone number field and you shouldn't have a problem anymore)

      @Sunflower Hi Eva, thank you so much for your amazing insight, I was really close to not get a Covid Test with me because the airline I am traveling with informed me that I did not need a test, and that my J&J vaccine was considered "fully vaccinated". So, by now I just have the J&J, that Perú does NOT consider as a "full vaccine", and the booster from their favorite Pfizer.
      Also thank you for the clarification on how to fill out the telephone numbers!
      Now, one more question: I just have a US cell phone number, but on the slots of telephone number (aka Land line) should I write the home telephone number in Peru where I am staying at? Thanks again!!

      I just followed a quite interesting discussion of a few travelers and foreigners living in Peru about the acceptance of the J&J vaccine. While everyone only having the one shot and a booster didn't have any problem entering Peru, the experinences when doing things in Peru were varied. Most said that no matter where they went or what they wanted to do they were first always denied entry and had to explicitly point out that the one shot J&J should be considered as 2 shots and the booster as their third shot. A few then were allowed to enter, most others still denied, sometimes a supervisor was called and then after some discussions either allowed or most often denied entry. One person stated that he has a vaccination card where the field for the first vaccination is crossed out, his one shot J&J was written in the second vaccination field and his booster in the third vaccination field. He claimed that he never has any problems. So, having the one shot and a booster is a mess in Peru and hit or miss.

      With your phone number you have two options: either put your cellphone number as well in the fields supposed for the landline number or just put the number of the place you stay in Peru. Honestly, it doesn't matter. No one will ever use the number anyway.

      @Beba Bode Hi Eva, I have one more doubt: regarding covid testing it says, "test result that is not older than 48 hours after being issued and before boarding the plane has to be presented". It may sound silly, but I am taking a local flight before boarding on the international flight. So, my international flight leaves 5 hours after the local flight. Which time frame should I consider? Many Thanks!
