How to fill in the online health form and get the Peruvian health affidavit

Until October 12, 2022 everyone - visitor, resident and Peruvian; adult and child; vaccinated or unvaccinated - who came to Peru by air was required to present a Health Affidavit when entering the country. This changed with the Ministerial Resolution 811-2022-MINSA.

So, since October 12, 2022, the Health Declaration is no longer required to enter Peru! The website to fill in the form has been deactivated as of October 20, 2022.

As Peru is still in a state of (national health) emergency and prevention and control measures to stop the spread of Covid remain in place at least until February 24, 2023, things can be tightened again in no time and the health declaration might be necessary in the future again. So, for now, we leave our guide to fill in the form below.

 

Completing the online form takes less than 10 minutes, is super simple, free of charge and at the end you immediately get the required health affidavit which you can either download or print. The form can be filled in up to 72h before entering Peru (within 72 hours prior to your arrival in Peru). You might be asked to show the health affidavit when checking in to your flight to Peru and/or at the airport in Peru and/or when traveling.

Please only use the official website for obtaining the health affidavit and don’t fall for a number of service providers offering to get the sworn statement for you while charging you anything from US$ 20 up to US$ 60 for something you can easily do yourself in a few minutes for free.

How to complete the online form and get the Peruvian health affidavit

If applicable, before you start switch off your VPN and any translation program that might run on purpose or automatically!!!

Step 1

Visit the official health affidavit website.

There have been reports that the page doesn’t always load correctly in some browsers, an error message pops up and people can't fill in the required fields on the first page. If you are using a VPN, switch it off, reload the page and it should work smoothly. Otherwise, try to use a different browser; we had no problems with Chrome, Firefox and Safari.

Additionally, we get many questions that people can't find nationalities or states or cities in the drop-down lists on the first and second page. Note that the names are always in Spanish (see Step 2)! If you see them translated, switch off any translation program that may run in the background automatically by your browser or intentionally otherwise error messages can pop up or you won't find the correct names.

Step 2

At the bottom of the first page you can choose between English and Spanish, however while the labelling of the spaces you have to fill in changes from Spanish to English and vice versa, the drop-down list under Nationality remains in Spanish; so if you are, for example, a US national you won’t find United States or US on the list, but have to click on EE.UU (Estados Unidos, the Spanish name for the US); or if you are German, click on Alemania, etc. Those from the UK will look in vain for United Kingdom but as well for Reino Unido; select instead Gran Bretaña  (Great Britain) or, if you are from England, you could choose Inglaterra, or, if you are from Scottland, Escocia.

Then choose the document with which you are travelling: DNI, CE or PAS (passport) and enter the number. Complete the first page by adding your birthdate and then click on “Search”.

Step 3

The second page of the Peruvian online form to get the health affidavit is divided into two parts:

  • Part I: Personal Information
  • Part II: Health information

So, first fill in your personal information exactly as in the document you are traveling with, your arrival date and flight number. Under “Country of origin” (the country from which the flight departs to Peru) the names are again in Spanish (see step 2).

Then choose your destination in Peru. You are asked for the department (Peruvian state), the province and the district; so, for example, if you stay in the district of Miraflores in Lima, choose Lima (department), Lima (province) and Miraflores (district) or if you travel to Machu Picchu choose Cusco (department), Urubamba (province), Machu Picchu (district). If you are traveling around, either choose your first destination or the place where you stay the longest.

Now add a contact phone number and a cell phone number. Be aware that fields for the (cell) phone number are Peruvian-style, so, if necessary, you might have to "adjust" your phone number to the Peruvian structure.

  • the country code must have 2 or 3 digits
  • the city / area code must have 2 or 3 digits
  • the phone number must have at least 6 digits, the cell phone number at least 8 digits

Example: let’s assume you have a US cell phone number which usually is structured as follows: +1 (country code) 123 (area code) 1234567 (number). This cell phone number won't be accepted as there are two problems with it: the system doesn’t allow the “+” and the number only has 7 digits. So, just enter in the country code field 01, in the city code field 12 (the first two digits of your area code) and in the number field 31234567 (last digit of your area code and then the 7 digits of your number). Problem solved.

Then enter a valid e-mail address you have access to even during your travel.

The following fields seem to be aimed at Peruvian travellers (and in an earlier version of this form were in a section which only Peruvians had to fill out) and don't really fit international tourists.

You are asked for your "real address", but you can only choose a Peruvian department (state), province and district. So, just as above, where you had to choose your destination in Peru, use the address of your hotel or the address of friends / family you are staying with. If you are traveling around, choose the first hotel you are staying at or the place where you stay the longest.

And the next field seems even stranger; you are asked to enter your "real address after isolation". This just makes little sense for foreign travellers, so just enter the street address of your accommodation.

The next fields aren't better as you should add the persons who are with you at your "real address". We recommend to either leave these fields blank (each person traveling should have his or her own health declaration anyway) or enter the information of whoever is traveling with you. After entering the data of a person, you have to click "Add", otherwise the system won't accept the entry.

Step 4

In Part II Health Information, as of May 2022, you just have to answer three Covid-related questions and then tick the box before the declaration of oath; the geolocation authorization for your cell phone, which never had a legal basis in Peru, is gone.

Click registrar, and you immediately get the Health Affidavit. At the top of the page, you can and, as once you leave this page you can't return, should download ("descargar") and/or print ("imprimir") the form. Sometimes, if you have selected "imprimir" the window that opens remains blank. Just shut the print window with the "x" at the top right, choose the "descargar" button and print from there.

    The form will not accept my contact phone number.  I have tried it just the 7 digits (US number), 7 digits with area code, 7 digits with area code and country code (tried country code as 1, 01, and 001).  It will not accept any format.  How do I fill in these fields?

      As described above, the (cell) phone number has to be entered in the way Peru writes its numbers. So, if necessary, you might have to split up your US number(s) to fit the Peruvian-style spaces.

      The country code has to have at least two digits and at the most three numbers; the city code as well at least 2 and not more than three digits and the phone number at least 6, the cell phone number at least 8 digits.

      If you follow these tips, the system will accept your phone number.

      Hello Ethan,

      For months everyone wanting to enter Peru had to present a negative PCR test result no matter if vaccinated or not. However since November 1, 2021 Peru requires an official vaccination certificate from everyone older than 12 years entering the country, proving that they are fully vaccinated against Covid-19 at least 14 days before arrival. Those not vaccinated must present a negative real-time COVID-19 molecular (RT-PCR) test result that is not older than 72 hours after being issued and before boarding the plane.

      See current Supreme Decree 179-2021-PCM, article 8.7 "Los peruanos, extranjeros residentes y extranjeros no residentes de 12 años a más cuyo destino final sea el territorio nacional, en calidad de pasajeros e independientemente del país de procedencia, deben acreditar el haber completado, en el Perú y/o el extranjero, su esquema de vacunación contra la COVID-19, catorce (14) días antes de abordar en su punto de origen; en su defecto, pueden presentar una prueba molecular negativa con fecha de resultado no mayor a 72 horas antes de abordar en su punto de origen."

      So, it seems to me that the information on VeriFly, which I personally never heard of before, is outdated. However, please check as well with your airline. Some of them might require a negative test despite your vaccination status.

      @Sunflower Hello I have a question. My flight is at midnight leaves Thursday. I was recently exposed to covid from a friend. However I have tested negative with a rapid and a PCR and my family is negative. Would I still be able to fly with a yes on the health declaration?

      Hello Amy,

      in your case answering the question on the Health Declaration if you had contact with any person ill from Covid, depends on your honesty (be aware that you declare under oath that the information provided by you is correct) .

      However, if you check "yes" you have to live with possible consequences: the airline might not allow you to fly despite your negative test result and even if you make it to Peru you might be put into quarantine upon arrival; or no-one checks your health declaration properly and you can just enter the country without anyone bothering you.

      If you lie and check "no", neither the airline nor the health officials at the airport can prove otherwise, if you arrive in Peru healthy and with a negative PCR test result.

      So, how you answer is up to your conscience.

      @Ethan The US does require international travelers to be fully vaccinated. Peru does not require it, but if you are not, you need a PCR test. That's what I know. Verifly is connected with American Airlines. They expect you to be vaccinated if you're an international traveler and a foreigner. I don't know if this required of American citizen.

      sorry for replying so late. I somehow missed your post.

      Did you manage to fill out the form? I have just tried it from my side with Firefox and Chrome and it worked without any problems. What error message do you get?

      Make sure you are not using a “VPN” and fill out all the required fields correctly as described above. Also note that the date in Peru is entered this way: dd/mm/yyyy. To further assist you, it will be necessary to know the error message you see.

      the form can be filled out up to 72 hours (so three days) before your arrival in Peru. So if you arrive on January 5 at - let's say - 02.00pm, the earliest you can fill it in is January 2 at 02.00 pm .
      As most airlines check that you have the health declaration when you check-in to your flight to Peru, best submit your data in the time frame before check-in and not more than 72 hours prior to your arrival time in Peru.

    Hello, I am going to Peru on the 31st. I want to make sure I know what I need. I have been vaccinated twice. I'll do an RT-PCR test on the same day of travel. I see that I have to do the affidavit above, and I believe I have to download the immigration app. Is there anything else I need or to show when I get to Peru? I want to make sure I can enter with no problem and want to know what's mandatory. Thank you.

      Seems to me you are well prepared. As things stand today you need:
      - certificate proving you are fully vaccinated or negative PCR test result to enter Peru
      - health declaration to enter Peru
      - pre-registration (not obligatory and often doesn't work)
      - vax certificate to enter closed spaces

      There is no reason to panic. You are all good. The Affidavit of Health should not be “older” than 72 hours prior to your trip to Peru – so the one you have filled out will do just fine. Just print it out and take it with you. Have a great trip!

    i was just filling out the jurada de salud! i was waiting until midnight over here to be sure with the 72hrs,then on the date i realized that it was still the 8th of november because of the time difference in peru!
    do you think if that can cause a problem at the check in?
    on top of that i inserted the contact nr.of my girlfriend and not mine

    do you think its possible to fill out another one?
    iam really not sure what to do now

      your girlfriend's phone number wouldn't pose a problem, but the date might; it shouldn't be more than 72h before your trip to Peru. So best fill in the form again and present the new one if you are asked at the check-in and / or just after landing at the control point of the Ministry of Health before immigration control.

      @Daniel You can fill out the form as many times as you need! The Peruvian authorities are only interested in the printed form and there is no record kept on how many times you have created it! Trust me, have done it many times and never had an issue…

      @Daniel Personally I think that even with your form now, you won't have any problems. It states 8th and you arrive on the 11th, no hour on the form, so you might be in the 72h time frame. And as TinTIn mentioned, you can fill in the form a second or even third time, it's not recorded anywhere. Most times the staff at the check-in and at the airport in Lima just wants to see that you have the form, but rarely really checks it.

    Hello, I am still a little confused about when it has to be filled: within 72hrs before entering Peru (the same like PCR test). Or, at least 72hrs before entering?
    Different websites state opposite answers, and I just want to make sure I understand correctly. Thanks !

      The current law and the official website of the Peruvian government state "dentro de las 72 horas antes de viajar", this means within 72 hours before your travel. So you can fill out the health declaration up to 3 days, 2 days, a day or an hour before your check-in.

      Additionally, Peru requires a negative real-time COVID-19 molecular (RT-PCR) test result that is not older than 72 hours after being issued and before boarding the plane; the supreme decree (see article 8.7) states "... deben contar con una prueba molecular negativa con fecha de resultado no mayor a 72 horas antes de abordar en su punto de origen". So, your test result can be issued up to 72 hours before your check-in.

    It was still giving me problems, even when I used a browser (Microsoft edge) that didn't want to pull some of the translation tricks, entered Lima, instead of "Lime", & etc,  When I got to the end and hit the green "Register" button, it gave me some sort of brief error message in Spanish, but didn't progress to giving me the "Declaration".  

    After much re-do, cursing of the website, my computer, Peruvian bureaucrats and web designers, I finally figured out the problem:  I put my wife's name in the entry for "people with whom you will be accompanied", but ignored the red "Add" button, assuming it was for adding the same entries for additional persons in my party...NOT SO; as soon as I finally tried that button, it created a separate little entry with my wife's info, and when I got to the end of the form, the REGISTER button responded with the completed form with options to save or print it.

    By the way, my suggestion is to save the form first, to make sure you don't lose the info and have to re-do it (ask me how I know), then print later.

    Also, the entry process no longer includes the oath about the negative PCR test.

      And thank you even more for pointing out the missing oath for the negative PCR test and the fields for the "accompanying person". When I checked the form four days ago to see if I have the same problems as you, the form was exactly as described above.

      Now, I checked again, because I didn't understand what you wrote about the "accompanying person" and the Add button - before these fields only had to be filled in by Peruvians who visit family or friends - and they completely changed the form. So I have my work cut out for me to update above information.

    This form is a real mess.  Sometimes, it doesn't show Lima in the department or province choices, it shows "lime", and if you accidentally select something else, it won't let you correct it in "department", even if you manage to get it to take "Lime" in the "province" box.  It wouldn't let me change the gender of my companion from male.  In the contact cell number box, you have to break up a standard US cell phone number to put a couple of digits into the "city code" box (won't let you leave it empty), and still have the required minimum eight digits to put in the cell number box. However, it will let you skip "city code" in the "contact phone number " boxes. In various places it will sometimes show USA (though just below Ecuador) but sometimes randomly use different abbreviations, like EE.UU ( I think it once offered me something like "ESU").  Sometimes the calendar drop-downs had some of the dates written out and running over adjacent numbers, rather than just showing as numbers.

    Thanks for the info, though.  It was otherwise difficult to find out exactly what was expected for some of the entries.  I'm hoping I can eventually get the entire thing filled out without it glitching

      after reading your comment, I was stunned and tried again to fill in the form on my desktop using Firefox and Chrome and on my iPad and Android phone. As described above, under step 2 in the two drop-down menus where you have to choose your nationality, I only saw EEUU (Estados Unidos in Spanish for United States) under Ecuador; the form as well only showed me Lima and not Lime; I could change the gender and province / department without any problem. However, fields for the phone number are Peruvian-style, so you just have to split up the numbers to fit the spaces.

      Anyhow, I wondered why you have these problems. Do you perhaps use a translation program or Is it possible that your browser automatically translates webpages in foreign languages. For me that would explain why you are shown "Lime" for "Lima" (typically for Google translate) and might explain other problems you have. If so, switch the translation function off and try again. Probably as well clear your browser cache and if you use a VPN switch it off as well.

      @Sunflower Yes, it was very strange, especially since it wasn't consistent, like varying the USA equivalent and sometimes showing Lima, sometimes Lime.  I did think maybe it was some type of auto-translate interference.  Didn't have my VPN on.

      Not yet inside the 72 hour arrival window, so was just trying it out.  Now that I know more about it, it should be easier & quicker...& I can switch to another browser to try, if necessary.  I may put in my Google voice number when finalizing it, so any potential contact messages will show up when I have internet access.

      Yes, as we mentioned under "Step 2", the drop-down list under Nationality is always in Spanish no matter if you fill in the Spanish or English version of the form. So, if you are a US national you won’t find United States or US on the list, but its Spanish name; just click on EEUU (Estados Unidos).

      Hello Stuart,

      According to the Peruvian government website, you can present the printed health declaration or the virtual one. I don't know if they accept a photo or if they even recognize that it's a photo and not the virtual declaration form when you show them the screen of your phone.

      As most airlines flying you to Peru already check the health declaration when you check-in, give it a try. If the airline refuses your photo, you can always fill in the form on your phone again and then have the download on your phone, which is allowed and accepted.

      Edit: I completely forgot, you could, for example, save the download of the health declaration form on your cloud or google drive or one drive if you are using such services and then access it on your phone. Or you could send the download from your laptop to your own e-mail which you hopefully have as well installed on your phone and have there the "original" virtual version.
