How to fill in the online health form and get the Peruvian health affidavit

Until October 12, 2022 everyone - visitor, resident and Peruvian; adult and child; vaccinated or unvaccinated - who came to Peru by air was required to present a Health Affidavit when entering the country. This changed with the Ministerial Resolution 811-2022-MINSA.

So, since October 12, 2022, the Health Declaration is no longer required to enter Peru! The website to fill in the form has been deactivated as of October 20, 2022.

As Peru is still in a state of (national health) emergency and prevention and control measures to stop the spread of Covid remain in place at least until February 24, 2023, things can be tightened again in no time and the health declaration might be necessary in the future again. So, for now, we leave our guide to fill in the form below.

 

Completing the online form takes less than 10 minutes, is super simple, free of charge and at the end you immediately get the required health affidavit which you can either download or print. The form can be filled in up to 72h before entering Peru (within 72 hours prior to your arrival in Peru). You might be asked to show the health affidavit when checking in to your flight to Peru and/or at the airport in Peru and/or when traveling.

Please only use the official website for obtaining the health affidavit and don’t fall for a number of service providers offering to get the sworn statement for you while charging you anything from US$ 20 up to US$ 60 for something you can easily do yourself in a few minutes for free.

How to complete the online form and get the Peruvian health affidavit

If applicable, before you start switch off your VPN and any translation program that might run on purpose or automatically!!!

Step 1

Visit the official health affidavit website.

There have been reports that the page doesn’t always load correctly in some browsers, an error message pops up and people can't fill in the required fields on the first page. If you are using a VPN, switch it off, reload the page and it should work smoothly. Otherwise, try to use a different browser; we had no problems with Chrome, Firefox and Safari.

Additionally, we get many questions that people can't find nationalities or states or cities in the drop-down lists on the first and second page. Note that the names are always in Spanish (see Step 2)! If you see them translated, switch off any translation program that may run in the background automatically by your browser or intentionally otherwise error messages can pop up or you won't find the correct names.

Step 2

At the bottom of the first page you can choose between English and Spanish, however while the labelling of the spaces you have to fill in changes from Spanish to English and vice versa, the drop-down list under Nationality remains in Spanish; so if you are, for example, a US national you won’t find United States or US on the list, but have to click on EE.UU (Estados Unidos, the Spanish name for the US); or if you are German, click on Alemania, etc. Those from the UK will look in vain for United Kingdom but as well for Reino Unido; select instead Gran Bretaña  (Great Britain) or, if you are from England, you could choose Inglaterra, or, if you are from Scottland, Escocia.

Then choose the document with which you are travelling: DNI, CE or PAS (passport) and enter the number. Complete the first page by adding your birthdate and then click on “Search”.

Step 3

The second page of the Peruvian online form to get the health affidavit is divided into two parts:

  • Part I: Personal Information
  • Part II: Health information

So, first fill in your personal information exactly as in the document you are traveling with, your arrival date and flight number. Under “Country of origin” (the country from which the flight departs to Peru) the names are again in Spanish (see step 2).

Then choose your destination in Peru. You are asked for the department (Peruvian state), the province and the district; so, for example, if you stay in the district of Miraflores in Lima, choose Lima (department), Lima (province) and Miraflores (district) or if you travel to Machu Picchu choose Cusco (department), Urubamba (province), Machu Picchu (district). If you are traveling around, either choose your first destination or the place where you stay the longest.

Now add a contact phone number and a cell phone number. Be aware that fields for the (cell) phone number are Peruvian-style, so, if necessary, you might have to "adjust" your phone number to the Peruvian structure.

  • the country code must have 2 or 3 digits
  • the city / area code must have 2 or 3 digits
  • the phone number must have at least 6 digits, the cell phone number at least 8 digits

Example: let’s assume you have a US cell phone number which usually is structured as follows: +1 (country code) 123 (area code) 1234567 (number). This cell phone number won't be accepted as there are two problems with it: the system doesn’t allow the “+” and the number only has 7 digits. So, just enter in the country code field 01, in the city code field 12 (the first two digits of your area code) and in the number field 31234567 (last digit of your area code and then the 7 digits of your number). Problem solved.

Then enter a valid e-mail address you have access to even during your travel.

The following fields seem to be aimed at Peruvian travellers (and in an earlier version of this form were in a section which only Peruvians had to fill out) and don't really fit international tourists.

You are asked for your "real address", but you can only choose a Peruvian department (state), province and district. So, just as above, where you had to choose your destination in Peru, use the address of your hotel or the address of friends / family you are staying with. If you are traveling around, choose the first hotel you are staying at or the place where you stay the longest.

And the next field seems even stranger; you are asked to enter your "real address after isolation". This just makes little sense for foreign travellers, so just enter the street address of your accommodation.

The next fields aren't better as you should add the persons who are with you at your "real address". We recommend to either leave these fields blank (each person traveling should have his or her own health declaration anyway) or enter the information of whoever is traveling with you. After entering the data of a person, you have to click "Add", otherwise the system won't accept the entry.

Step 4

In Part II Health Information, as of May 2022, you just have to answer three Covid-related questions and then tick the box before the declaration of oath; the geolocation authorization for your cell phone, which never had a legal basis in Peru, is gone.

Click registrar, and you immediately get the Health Affidavit. At the top of the page, you can and, as once you leave this page you can't return, should download ("descargar") and/or print ("imprimir") the form. Sometimes, if you have selected "imprimir" the window that opens remains blank. Just shut the print window with the "x" at the top right, choose the "descargar" button and print from there.

    i have just completed the affidavit for myself and  added 1 person who i am travelling with, will they need their own affidavit or are they able to travel on mine?

      yes, each person needs his/her own form even though you added him/her to your form.

      So, to be frank, adding a person to your form doesn't make sense and, as often error messages pop up, doing so makes the whole thing more complicated than necessary.

    Hello, I currently have positive covid and about to get my recovery certification on JULY 5th. I have a flight to Lima on JULY 7th, I also I have been vaccinated three times . Will I be able to enter Peru ? Do I need to declare I have been to quarantine in the last 14 days ? Will I be able to enter with that declaration ?

      Yes, you can enter Peru. How you fill in the Health Declaration is up to you, but be aware that you affirm in lieu of oath doing it correctly.

      Anyway, you could lie on the health declaration and not mention your quarantine, and just show your vaccination card proving 3 shots. Neither the airline nor the health officials at the airport in Lima can prove otherwise.

      Or you are honest and declare your quarantine. You will still get the Health Declaration. I don’t know how the airline reacts to this, but as you have a recovery certificate, there shouldn’t be a problem. Entering Peru most probably no-one will bother how you filled in the form, at least if you don’t have any symptoms (if you are still sick, you could be forced to take a Covid test and might be put into quarantine). So, if you are healthy expect to just present the form which will end up on a pile of hundreds of other and to show your vaccination certificate.

    We will soon be arriving in Callao on a cruise ship. We have a question about that.
    After disembarking we will drive straight to the airport and fly to Quito.

    Is a Peru-Affidavit also required in our case? Thank you for any info?.

      You only need the health declaration if you travel to Peru by air; if crossing a land border into Peru, you don’t need it and, even though not explicitly mentioned in the regulations (probably because they were written when ports and land borders were still closed), I assume you neither need it when entering the country via a port. In case I’m wrong, I’m sure the cruise line will inform you early enough so you could still fill it in.

      You do not need the form when traveling in Peru and for leaving the country.

      However, be aware that air passengers flying into Ecuador must fill in the Ecuadorian health declaration form.

      But as I already mentioned in my reply yesterday, no-one will bother if the country of origin is the one bringing you directly to Peru or the one where you started your trip.

      Yes, you are right. There is no US or United States in the list of countries.

      As described in step 2 and 3 in our article above, the drop-down lists under Nationality and Country of Origin remain in Spanish even though you fill in the English form; so, if you are looking for the US, click on EE.UU (Estados Unidos, the Spanish name for the US).

      FYI, I just finished doing ours and it had USA for an option now. In one place, after entering USA it changed to USES, oddly. Question: I think I may have put our arrival date wrong on 1 person's in our group but there's no arrival date on the QR printed one, just a signed date..should I redo that person's? It would only off by 1 day (we arrive tomorrow and I think I may have forgotten to change it from the automatic 29th that was there)

      Great that you are nearly on your way.

      But where did you have USA on the list? I just checked and couldn’t find it. No USA under U, just EEUU under E (see screenshots below). Are you possibly using a translation program? I’m glad that everything else worked, and you got your health declarations.

      Anyway, personally I think, it’s not necessary to redo the one declaration with the possibly wrong date. But, as you told me before that you are paranoid, better fill it in again with the correct arrival date, so everything is fine and you don’t have to worry.

    My health affidavit isn’t being processed. Responds in Spanish:  hasta 72 horas antes de su vuelo, 
    my flight departs Friday, July 1st at 8:24 PM evening. 
    Aren’t I ok with the 72 hrs?

      When does your flight arrive in Peru? You can only fill in the form the earliest 72h before your arrival (!!!) in Peru. So, not sure where you are coming from, but I think you filled in the health declaration too early and that's the reason for the error message.

      Additionally, sometimes the system has a hiccup, especially with flights arriving late at night. So, if your flight, for example, arrives at the airport at 11.40 pm on July 1, once in a while the system will give you an error message even if you try to register around 11.40 pm on June 28.

      So, wait until tomorrow, clear your cache or use another browser and fill in the form again. It should work without any problems tomorrow.

    Hello Eva,
    I have just finished filling out all the info on the Health Affidavit and when I click REGISTRAR it says I need to do it 72 hours before my trip and it is 72 hours before my trip, do you know what must be going on?  I've done through my cell and through my computer and same thing. Any recommendations please? Do you think it is giving me a message due to something I did wrong? even though I have followed your instructions to the T???? Please respond. Thanks! Maria

      yes, this sometimes happens. I haven't got a clue why. And no, nothing you can do wrong. It's the system that for whatever reason sometimes seems to get confused with the date.

      Just wait until tomorrow, then clear your cache or use another browser and it should work fine.

I google my question as well and it says also 72 hours before arrival to Lima, so now I am more confused.  I thought it was 72 hours from the time of departure from LAX.  Of course the arrival to Lima-Peru is different more hours.  What do you think, does it make sense to you?

      As described above in the article, you can fill in the health declaration within 72 hours prior to your arrival (!!!) in Peru. Not sure why you are confused, that’s how it is and always was. But you have to fill in the form before your check-in, as airlines usually check that all passengers have it.

      So, let’s assume your flight lands at 10.00 am on June 25 at Jorge Chavez International Airport. The earliest you can fill in the form is 10.01 am on June 22. You can do the math for your flight.

      However, sometimes the system has a hiccup, especially with flights arriving late at night and when you fill in the form late in the evening. So, if your flight, for example, arrives at the airport at 11.40 pm on June 24, once in a while the system will give you an error message when you try to register at 11.50 pm on June 21. Or the same if you try to fill in the form at 10.30 pm. The only solution is to wait until the next day, and it usually works fine then.

      Not sure which browser you use; usually it’s Safari if you are using Apple devices, Chrome for Android and Chrome or Edge (or others) on your computer.

      If you only have one internet browser installed, you either have to install another one or to make it easy just clear the cache, so the system “forgets” the data you entered and the error message.

    Question: After clicking "registrar" I will immediately get the document to be printed according to your instructions.  Will this document HEALTH AFFIDAVIT for me, be sent to my email? or do I need to print it right away? I will be traveling the day after tomorrow, would you please answer my question at your earliest convenience?       first of all I deleted your Spanish comment and combined your other two comments.

      We usually answer all comments send to us within 24h; I replied to your comments from June 1  less than 15 minutes after you send them. You find the answer here on the page and additionally got a confirmation via e-mail (if you used a valid e-mail address). So, if you scroll down a bit you will find your question and my answer to it or check your e-mails.

      Anyway, as described above, yes, as soon as you click on "registrar" you immediately get the Health Declaration on your screen. It won't be sent to you. You have to download ("descargar") and/or print ("imprimir") it which can be done with the buttons at the top of the page.

    Do you know if we have have been in COVID isolation in the last 14 days, we won't be allowed to enter Peru? I recently had COVID, isolated accordingly, and have tested negative and am okay to travel (per my doctor's note). But I'm concerned that checking yes is going to automatically disqualify me from entry! Thank you for any insight you might have.

      No, checking “yes” won’t disqualify you from entering Peru. Most probably no-one will even check your health declaration form properly. And if, you have proof that you are fine and fit to travel.

    I thought the section that says country of origin meant the country you are born in not the country you are traveling from? Am I incorrect? If so can the affadvit be redone?

      the flight which brings you directly to Lima; so in your case, the flight from Panama City to Lima.

      So, under country of origin, select Panama and then the flight number of your Panama City - Lima flight.

    Can you help with this situation.  A friend of mine is in Chile and wants to go to Peru.  He tested positive for covid and completed the required quarantine, has no symptoms and now has a document from the Chilean health authority saying he is done and can end his quarantine.  If he fills out this form and checks "yes" to having quarantined in the last 14 days, will he still be able to get the affidavit or will another question pop up?

      Yes, even if you check "yes" you can get the health declaration. The question is what happens when you have the yes on your form.

      Probably the airline will ask about it or the staff of the Peruvian Ministry of Health when entering Peru. Then your friend should just show the document. Not sure how the reaction will be. Or both won't bother.

      Is he vaccinated? And had a negative test? Best have both things on hand as well.

    Hi, I will stay in San Borja district, so I put down Lima as department, Lima as province, and San Borja as district. I hope that is correct. 

    For telephone numbers, I was a bit unsure if I should put down my own home phone number in Sweden, or the phone number in the place I will stay at in San Borja.  I think I will put down the San Borja phone number. Which city code should a write for that?

      Lima, Lima, San Borja is correct.

      If you use the landline number of your accommodation, the country code for Peru is 51 and the city code for Lima is 1 (as you need two digits 01). If you use a cell phone number of where you stay in the landline field, no 1, just the first three digits of the number.
